*An email address is required to submit an online application. Bring required documents and visit the Public Works Counter located inside San Mateo City Hall (330 W. 20th Avenue, San Mateo, CA 94403) to submit an application without an email address.
Upload a proof of residency document. Acceptable documents includes a current driver's license, current DMV vehicle registration, utility bill, lease agreement, car insurance policy, or pre-printed personal check with the resident's name and address in a permit program area.
